Our Admissions Director will coordinate the transition from the Hospital to our facility, working with the case managers, social workers, physicians and other health agencies.

Our skilled professionals assist residents and their families with the Medicaid application process. For your convenience Susanna Wesley accepts evening and weekend admissions.
